I, (the person participating) the undersigned (“THE PLAYER”) assumes all risk of personal injury which may result from participation in all activities of Erjy Fun Puck, Erjy Fun Puck Hockey Tournament or related activities.
THE PLAYER hereby releases, indemnifies and holds harmless, Erjan Remzi, and all staff, officials, employees and/or volunteers of Erjy Fun Puck Hockey and the owner or proprietor and employees of any ice facility used by Erjy Fun Puck, from any and all liability, claims, or actions for any injury which THE PLAYER may sustain while participating in any Erjy Fun Puck activity.
THE PLAYER understands and agrees that the sport of ice hockey has physical dangers which may result in serious injury or death. THE PLAYER is advised to carry medical insurance. THE PLAYER certifies that he/she has no known medical condition, physical or mental including previous concussions which would prohibit him/her from playing the sport of ice hockey. Should THE PLAYER have sustained any previous injuries the participant confirms, they have been assessed by a medical professional/doctor and have been approved to participate in the sport of ice hockey.
I agree to allow Erjy Fun Puck Hockey and its related companies to use the participants names and or pictures for advertising purposes.
Players participating in Erjy Fun Puck Hockey must be wearing full hockey equipment. This includes a CSA-approved hockey helmet.
Please refer to the Hockey Canada website for a complete list of hockey equipment. Players will be removed from the ice should they not have the proper equipment.
